Intake Assessment – Orientation

An Intake assessment must be completed with each of the parents prior to the beginning of service. Parents must be interviewed separately and at different times so that they do not come into contact with each other.

​

During the intake process, you will be asked about the reasons you are in need of Supervised Visitation and information on any family issues that may impact the parent/child contact or the safety of the participants. You will be asked about ongoing or chronic medical conditions of the participants that could affect the health and safety of the child, or the parents, or other participants during parent/child contact.

Parents will be given written information about preparing their children for supervised visitation services prior to the first visit and in accordance with the child's age and stage of development.

​

The supervisor will explain the program rules and policies with each parent prior to the beginning of service. During the Intake appointment, the supervisor will inform each parent about the limits of confidentiality and the service agreement must be signed by each parent prior to the commencement of services.

 

 

Please bring the following items to the Intake meeting:

  â–¡ A copy of this packet.

  â–¡ Copy of Photo ID of yourself and anyone you would like to be able to pick-up or drop off your children.

  â–¡ Copy of Court Orders, Stipulations, or Legal Documents that pertain to supervised visitation (you can email these documents to angeladoble@yahoo.com ahead of time if they are large files).

  â–¡ Copy of Restraining order, no-contact order, or stalking order (if applicable).

  â–¡ Recent individual photos of your children who will participate in the visitation. 

  â–¡ Any additional information you believe would be helpful in order to provide safe and effective visitation.
